Sella, RN is our dedicated nurse to care for our patients with special needs,  chronic conditions, and recent hospital stays.  Don't be alarmed if she calls, she tries to call anyone that needs a little more monitoring. 

CHRONIC CARE MANAGEMENT (CCM)

Our organization offers a comprehensive and team-based approach to chronic care management (CCM) offered by Medicare and other insurance. Our dedicated CCM Nurse, Sella, reaches out to patients with two or more chronic conditions to ensure they receive the best possible care, ultimately reducing the need for urgent care visits and hospital stays.  

We make regular calls and conduct telehealth visits with our patients as needed to manage care. During these interactions, we check on your symptoms and ask basic care questions that are tailored to your specific medical needs. Some of the questions we may ask include: How are your symptoms? When is your next appointment with a specialist? Do you have an adequate supply of medications? Are you taking them as prescribed? Additionally, we inquire about any challenges you may face regarding accessing food, transportation, or other unmet needs.

If you experience any changes in symptoms, Sella will promptly schedule an appointment with your healthcare provider or specialist to ensure you receive the necessary care. Similarly, if you are running low on medications, Sella will either arrange for you to see your provider or specialist or assist you in refilling your prescriptions. Furthermore, if you have any unmet needs, we can help connect you with organizations that can provide the necessary support.

 As part of our comprehensive care approach, we also have a nutritionist available to assist you in managing your diet based on your specific condition. This additional resource ensures that your dietary needs are addressed and integrated into your overall medical care plan.

Remote Patient Monitoring (RPM)

We offer Remote patient monitoring (RPM). This allows patients with acute illnesses and chronic diseases to be monitored from their homes, and providers can make care decisions based on real-time information. We have blood pressure cuffs and scales, and we monitor both Dexcom and Freestyle libre glucose monitoring systems that we monitor Monday-Friday. Our Chronic Care Medical Assistant views blood pressure, pulse, and weight data when patients use their devices to send information via cellular capabilities.

We have Family Medicine, DO Residents, and Family Nurse Practioner students working with our providers, training to give excellent, quality care in a family practice setting.

What does this mean to you?

Our DO Residents are general practitioners and medical students finishing a few years of specialty training in family medicine.  Our Nurse Practitioner Preceptees work closely with James Rowley, FNP, to learn about care for all ages.

As always, Dr. Wilde is the eyes and ears, reviewing and overseeing patient care, keeping our quality of care at the best level possible. He is very active in the learning processes and supports all our staff.
